I, the undersigned employee, acknowledge that I have received, reviewed, and understand the policies, expectations, and responsibilities outlined during the BMS New Hire and University Site Orientation. I understand this information applies to my current role as a custodial team member. By signing below, I agree to comply with the following responsibilities as part of my employment with BMS: 1. I will maintain professionalism at all times and avoid saying or doing anything that may discredit the company. 2. I will communicate any concerns or issues directly with my supervisor in private and will not take performance feedback personally. 3. I understand that I am responsible for fulfilling the Scope of Work (SOW) assigned to me, including restrooms, classrooms, dorms, and common areas at university job sites. 4. I will follow all break and lunch regulations, including taking a 10-minute paid break every 4 hours and a mandatory 1-hour unpaid lunch when working over 5 hours (per California Labor Code §226.7 and §512). 5. I understand that pay is issued on the 1st and 15th of each month, and that pay rates are based on roles and responsibilities and should remain confidential (per California Labor Code §204). 6. I will complete the hiring process, including submitting a valid ID and Social Security Number, to ensure timely payment. 7. I will only use company-approved cleaning chemicals and follow all safety procedures in accordance with Cal/OSHA (California Labor Code §6401.7). 8. I will sign in and out accurately using the method provided and understand that falsifying time records is prohibited (California Labor Code §1174(d)). 9. If lodging is provided, I will respect shared hotel rooms and acknowledge that lunch is provided, but I am responsible for my own breakfast and dinner (California Labor Code §2802). 10. I will bring my own water jug and remain hydrated throughout the day. 11. I understand that I am responsible for arranging transportation to the job site and must be punctual.
